Please notify us at least 24 hours in advance through our patient portal or by calling the office during business hours at 480-568-7180. Cancellations with less than 24 hours' notice is subject to a cancellation fee of $40.00.
Insurance & Billing

Yes, no-shows and late cancellations (under 24 hours) will incur a $40 fee. See our full policy in your new patient paperwork.
Sorry, we’re not able to accept Medicaid or AHCCCS — this includes cash-pay arrangements for those patients as well.

Prescription Refills & Medication
In some cases, yes—especially for controlled medications or if you haven’t been seen in a while. Contact us to confirm.
Yes. We can e-prescribe to most local and mail-order pharmacies.

Patient Portal & Health Records
The portal gives you secure online access to your health records, visit summaries, lab results, appointment requests, messaging, and bill pay.
Nope! Access to our Portal and the healow app are completely free. We share this information with our patients to give them access to their health records and help them make better healthcare decisions.
Yes, the information is completely secure. It is protected with a username and password / PIN number of your choice and through SSL encryption.
We share information with our patients that we feel can help them and keep them informed. Every practice is different, and we try to provide patients with key information regarding their health. For a complete list of features, ask our friendly staff.
User support is built into the web portal and the app, or you can ask a member of our staff.
The app and our Portal allow you to reset your username and/or password. You can also call the front desk at 480-568-7180 and we can reset the username and/or password for you
Sign-up is simple and outlined in the steps provided on the Patient Portal login/signup page. You should receive a username and password from our practice via email that you can print out and customize after your initial login. Access can be made via any web browser,or through the smartphone app.
Yes. Secure messaging is available for non-urgent communication.
You can download your records through the Patient Portal or request a copy through our front desk.

Our current hours are: Mon–Fri: 8:00 AM – 5:00 PM. We’re closed on weekends and major holidays.
