If you'd rather take care of your forms in person, we have a private digital kiosk available in the office. Just be sure to arrive at least 45 minutes before your scheduled appointment so you’ll have plenty of time to complete everything without feeling rushed.

What to bring to your first appointment

To help us get to know you and provide the best possible care, please bring the following with you to your first visit:

  • Photo ID
    A government-issued ID (like a driver’s license or passport) to verify your identity.
  • Completed New Patient Forms
    Save time at check-in by filling them out in advance:

  • List of Current Medications
    Include prescriptions and dosing, over-the-counter medications, and any supplements or vitamins you take regularly.
  • Insurance Card
    So we can accurately bill your visit
  • Medical Records (optional but helpful)
    If you’re transferring your care from another provider, please bring copies of your recent medical records or have them sent to us before your appointment. Keep in mind that if you request the records yourself, you may be charged a fee. To avoid this cost, we’re happy to request them on your behalf at no charge—just let us know!
  • Questions or Concerns
    We encourage you to jot down any questions you have so we can be sure to address them during your visit.
